Mileage Reimbursement Forms

Because Heartland Christian School (Elementary) is accredited by the State of Iowa, you may be eligible to receive reimbursement money for transporting your own child to school.  Please fill out the forms below and return them to your local school district.  A list of school districts and their addresses may also be found below for your convenience.

* Deadlines for submitting reimbursement forms:
December 1 (first semester reimbursement)
May 1 (second semester reimbursement)

This reimbursement only applies to students in grades Kindergarten through 6th grade.

Note:  In the blank for "miles to school" put 2 miles or more.  DO NOT put less than 2 miles.
